Innate Spellcasting. The Possessed’s spellcasting ability is Charisma (spell save DC 17, +9 to hit with spell attacks). The Eye can innately cast the following spells, requiring no material components:
At will: cause fear

3/day: detect thoughts, misty step, suggestion

1/day: counterspell

Limited Magic Resistance. The Possessed has advantage on saving throws against spells and other spell effects cast by creatures within 15 feet of her. Also, when the Possessed damages a creature that is concentrating on a spell, that creature has disadvantage on the saving throw it makes to maintain its concentration.

Sneak Attack. Once during her turn, whenever the Possessed hits a target using her rapier attack (or any weapon attack using her Dexterity modifier), she may deal an extra 21 (6d6) damage of the same type to the enemy. To benefit from this feature, the attack must meet the requirements of the Rogue Sneak Attack feature or the Swashbuckler archetype Rakish Audacity feature.

Unhinged Nightmare. At the beginning of the Possessed's turn or any time she receives psychic damage, roll a d100. If the roll is equal to or below one-third of the total amount of her missing hit points, she suffers a momentary mental breakdown and the Eye momentarily takes over. Her turn immediately ends. Until the start of her next turn, the Possessed’s AC becomes 16, she loses her damage resistances, and she cannot actively use any abilities or reactions. While in this state, if a creature agitates the Possessed, she must perform three tendril attacks against the creature. These attacks do not use a reaction. The Possessed is agitated when an attack or spell effect originating from a creature the Possessed can see hits the Possessed. If a creature begins their turn within 15 feet of the Possessed, should they move or use an action or bonus action, they must succeed on a DC 13 Dexterity (Stealth) check or agitate the Possessed. If the target is outside of the attack's reach, the Possessed moves to an empty space within 15 feet of the target before performing both attacks. This movement does not provoke opportunity attacks. If she ends the movement while adjacent to a surface, she clings to that surface as if under the effects of spider climb until she moves. If she ends the movement in mid-air, she takes no fall damage from the subsequent fall. The Possessed cannot attack the same creature using this feature again until the start of her next turn.


Multiattack. The Possessed makes three attacks: two with her rapiers and one with her tendrils.

Rapier. Melee Weapon Attack: +9 to hit, reach 5 ft., one creature. Hit: 9 (1d8+5) piercing damage. When a creature is targeted by this attack, the target can't make opportunity attacks against the Possessed until the end of her next turn.

Tendrils. Melee Spell Attack: +9 to hit, reach 15 ft., one creature. Hit: 19 (2d12+5) psychic damage. If a target is hit, they become vulnerable for 1 minute. If a vulnerable creature is hit by the tendrils attack, they must make a DC 17 Wisdom saving throw or be affected by the confusion spell until the start of The Possessed's next turn.

Legendary Actions

The Possessed can take 3 legendary actions, choosing from the options below. Only one legendary action can be used at a time and only at the end of another creature's turn. The Possessed regains spent legendary actions at the start of her turn.

Erratic Footwork. The Possessed makes a rapier attack and moves her speed without provoking an attack of opportunity, in any order.

Disturbing Gaze. The Possessed chooses a target she can see within 120 feet who can also see her. The target must make a DC 17 Wisdom saving throw or become frightened until the start of the Possessed's next turn. If it fails by 5 or more, the target also becomes stunned.

Writhe (Costs 2 Actions). The Possessed makes a tendril attack against two creatures within 15 feet of her.

  • Minor changes to stat values to match modifiers or simply look neat.

  • Removed associated character name from document (title, ability descriptions, etc.) due to fear of being caught by players; all replaced with “The Possessed” for ambiguity.

  • Changes to Saving Throw proficiencies

    • Replaced CHA save prof. with INT save prof. to preserve Rogue stats

  • Revamped spell list:

    • Removed teleport from 1/day spells.

    • Moved counterspell from 3/day spells to 1/day spells.

    • Moved detect thoughts from at-will spells to 3/day spells.

    • Added cause fear to at-will spells.


  • Removed Magical Weapons feature

    • Redundant against target party, as well as the tendrils attack being the main source of damage anyway.

  • Changes to Unhinged Nightmare feature

    • When under the effect of this ability, the Possessed also loses resistances, encouraging players to attack it and reducing the likelihood of players wasting a turn waiting for it to end.

  • Removed Disarm action

    • Attack bonuses are too high, so the likelihood of player failure is too big.


  • Changed pronouns for the Possessed

    • Originally referred to as “it” and “she” interchangeably, now simply as “she.”

  • Changes to defensive abilities

    • Adjusted armor class from 18 (12 [studded leather armor] + 1 [dual wielder feat]) to 21 (10 + 5 [Dexterity modifier] + 6 [Charisma mod])

    • Adjusted resistances from bludgeoning, slashing, and piercing against non-magical weapons that aren’t silvered to bludgeoning, slashing, and piercing

  • Changes to features

    • Sneak Attack damage increased from 17 (5d6) to 21 (6d6)

    • During Unhinged Nightmare, The Possessed’s armor class is reduced to 16 and it loses its resistances.

      • Also, clarified wording on what happens if Unhinged Nightmare’s movement leaves it on a space beside a wall, the ceiling, or in mid-air.

  • Changes to attacks

    • When a creature is hit with the tendrils attack, roll a d6. If the number rolled is odd, the creature must make a saving throw against the confusion spell. If the number rolled is even, the creature must make a saving throw against being paralyzed.

      • Carried over Yeenoghu’s ability to paralyze with his flail attacks


  • Changes to stats

    • Constitution reduced from 19 to 17

    • Charisma reduced from 23 to 20

      • Armor class reduced from 21 to 20 (10 + 5 [Dexterity modifier] + 5 [Charisma modifier])

    • Proficiency bonus reduced from +7 to +4

      • Spell save DC reduced from 21 to 17

  • Changes to actions and reactions

    • Nerf to tendrils attack by reducing damage die from 3d12 to 2d12 and removing paralysis effect
